I did try the Xbox360 controller emu thing but it doesn't work in Vista 64 bit, and the readme instructions were obviously made by someone who's main language isn't english - My computer will not let me replace or delete XInput9_1_0 no matter WHAT I do though, so that could be the problem also. Create a text file named 'commandline.txt' and place it into the root of GTAIVs installation directory. I tried the -usedirectinput command line plenty of times.

The latest patch (1.0.2) totally disable my controller, which was working perfectly before the patch. Would be nice if Rockstar actually released a patch that fixed more than it screwed up.
